Assistance with Hospital and Rehabilitation Discharge Planning
Upon discharge from the hospital, many decisions need to be made regarding an elder’s returning home or ta a rehabilitation center for a short-term stay. Our Care Managers can ease the stress of this process by discussing the pros and cons of local facilities and identifying the differences among them. Since all facilities are different, we help the family understand what questions to ask and what to look for to ensure the right fit and best services for our client.
If an elder is able to return home, we ensure continuity of care by meeting with hospital personnel and arranging for necessary supportive services the elder might need.
Patient Advocacy
As an extension of your own family, our Care Managers continually look out for an older adult’s best interests, serving as an advocate and mediator. Care Managers can serve as a single source for clients’ legal documents, insurance information, and important medical documentation. When dealing with hospitals and doctors, we advocate for the elder by listening to what medical advice is given and communicating any changes to family members and caregivers. Our Care Managers serves as the line of continuity, ensuring that medical recommendations are carried out.
