Health concerns often become paramount. Children and other family members find themselves trying to make important decisions in a crisis. This can be overwhelming for those in the "sandwich generation" caring for aging parents, while caring for their own children.
JFS Elder Care professionals are geriatric care management specialists in assisting families coping with these issues. We offer these services to elders and their families:
- Comprehensive in-home assessments
- Liaison to families - local and distant
- Information and referrals for families and caregivers
- Family intervention
- Ongoing case management: coordination and monitoring of professional and community services
- Counseling for individuals and families either in our offices or in your home
- Caregiver Support Groups
- Individualized assistance with transitional living arrangements
- Serve as Guardian or Conservator
- Crisis intervention
